Personal details we process
Zi Yuan Ting processes your personal details when you use our services and/or because you have provided them yourself. 
We process the following personal details: 
-Initials, last name and given name
-Gender
-Date of birth
-Address
-Phone number
-Email address

Special and/or sensitive personal details we process
We, as caregivers, are required to keep a medical record under the Medical Treatment Agreement Act (WGBO). We will record the following details in your medical record: 
-Your care need and main complaint
-Relevant medical details such as diagnostic details, details regarding treatment and your medical history
-Relevant social details (for example relationship, children, life and work situation, hobbies, sport)
-Contact details of your general practitioner (‘huisarts’)
For minors younger than sixteen years old:
-Name, address, phone number and email address of both parents

Why and on what basis we process your personal details
To ensure a good treatment, it is necessary for any treating therapist at Practice Zi Yuan Ting to keep a medical record. This is a legal obligation under the Medical Treatment Agreement Act (WGBO). Your medical record contains personal details, notes about your state of health and details about the medical examinations and treatments performed. It also contains details that are necessary to your treatment and that we, where applicable and only with your explicit consent, have obtained from another caregiver, such as your general practitioner. 


The goal of processing personal details:
-Drafting a medical record on which your treatments are based. 
-Drafting an invoice for your health insurer. The invoice you will receive states the details your health care insurer requires, so you will be able to be reimbursed by your health insurer. Your name, address and hometown, date of birth, date of your treatment(s), performance code and a description of the treatment. 
-Keeping in touch with you regarding your treatment, invoicing and/or making appointments.

-Being able to perform a fitting treatment

Using your personal and medical record details 
The details from your medical record can also be used for the following purposes:
-Informing other caregivers (exclusively with your explicit consent), for example when your therapy has been completed or for a reference to another caregiver;
-For the use of an acting practitioner when your own practitioner is absent;
-For anoniniminised use during intercollegial assessment;
-For financial administration: the invoice contains your NAW-details and date of birth so are able to submit it to your health insurer;
-Your email address can be used for sending invoices, making appointments and asking questions regarding your treatment. 
Your therapist will do the utmost to ensure your privacy, consistent with the General Data Protection Regulation (AVG):
-by handling your personal and medical details with care
-by making sure unauthorised persons do not have access to your details. 
Agreements about this are made with for example colleagues and measures are taken to keep your details safe. 


Access to your record
Your practitioner, any intern present (with your consent) and the acting practitioner in case of the absence of your own practitioner (with your consent) are the only ones who have access to the details in your medical record. They are all under legal confidentiality (professional secrecy). 
A small part of your record (name, address, hometown, date of birth) can be given to an authorised person for (financial) administration (for example to provide invoices). 


Retention period
The legal retention period of your medical record is fifteen years. 


Sharing your personal details with third parties
Zi Yuan Ting provides your details to third parties only when this is necessary for the execution of our agreement with you or to comply with any legal obligation. 


Seeing, changing or deleting details 
-at the intake, we will ask your permission to record and process your details;
-we record your consent in your record;
-we do not record more details than necessary to accomplish the defined objective; You have the right to see, change or delete your personal details. You also have the right to withdraw your potential consent for processing your details or to object the processing of your personal details by Practice Zi Yuan Ting. You have the right to data portability. This means you can request us to send your digital personal details to you or another organisation, named by you. 
You can send your request for inspection, correction, removal, data transfer of your personal details, request to withdraw your consent, or objection to the processing of your personal details to Zi Yuan Ting Clinic. Zi Yuan Ting would like to remind you of the possibility to file a complaint about how we process your personal details with your practitioner or with the Autoriteit Persoonsgegevens www.autoriteitpersoonsgegevens.nl/en . 


How we secure your personal details 
Zi Yuan Ting takes the protection of your details very seriously and takes the necessary measures to prevent abuse, loss, unauthorised access, unwanted disclosure and unauthorised changes. We keep a digital record of our clients, which is secured with two-stage authentication and data-encryption. If you are under the impression your details are not secure or if there is any evidence of abuse, please contact us. 


Cookies, or similar techniques, we use: 
Zi Yuan Ting uses only analytical cookies that do not compromise your privacy. A cookie is a small text file that is saved to your computer, tablet or smartphone when you first use our website. The cookies we use are necessary for the technical functioning of our website and your user convenience. They make sure the website functions as it should and remember for example your preferences. We are also able to optimize our website this way. You can opt out of cookies by configuring your internet browser to no longer save cookies. You can also delete all previously saved information through your browser settings. 
We do not use cookies relating to advertising.
